This referral pad can be used by any healthcare professional. to make referrals for initial assesssments, re-assessments or adjustments easier. Referrals can be sent with patients who have scheduled an appointment or faxed directly to our office at 905-602-0650.

We will follow-up with patients who have been referred for orthotic services directly, following receipt of your referral.
